Wastewater-based epidemiology (WBE) has emerged as a powerful population-level surveillance approach for monitoring public health threats beyond traditional clinical reporting systems. By analyzing biological and chemical markers excreted by communities, WBE enables near–real-time assessment of disease circulation, antimicrobial consumption, and the spread of antimicrobial resistance (AMR). At a broad level, this approach offers a cost-effective, non-invasive means of capturing health signals across entire populations, including asymptomatic individuals and underserved groups often missed by healthcare-based surveillance. This article examines the operationalization of wastewater-based epidemiology as an early warning system for detecting antimicrobial resistance and community-level disease trends. It explores methodological foundations, including sampling strategies, molecular detection techniques, data normalization, and interpretation frameworks required to translate wastewater signals into actionable public health intelligence. Particular emphasis is placed on the detection of resistance genes, pathogen load dynamics, and temporal trend analysis, highlighting how WBE complements clinical and laboratory surveillance systems. The discussion further narrows to implementation challenges and governance considerations, such as standardization of protocols, integration with existing surveillance infrastructures, data quality assurance, and ethical management of population-level health data. Case-informed insights illustrate how operational WBE systems can support timely intervention, guide antimicrobial stewardship, and enhance outbreak preparedness. By synthesizing technical, operational, and policy dimensions, this work demonstrates that effectively operationalized wastewater-based epidemiology can strengthen early detection capacities, improve situational awareness of antimicrobial resistance, and support more proactive, data-driven public health decision-making at the community level. These findings underscore the strategic value of WBE for integrating environmental surveillance into resilient, forward-looking public health systems worldwide, particularly in contexts facing rising antimicrobial resistance and increasing pressures on conventional disease monitoring infrastructures and response capacity.
